Vaccine Locations & Registration for Second Dose Appointments
Registrations and appointments are currently available for residents due for a second dose. Please review each vaccination site listing for its appointment eligibility. Do not attempt to register if you do not meet this qualification. Appointments are Park and Walk In.
Walk-ins are now available at County-operated vaccination sites. Walk-in individuals will not need to bring any additional information to the site, however be prepared to register on site, which will include answering health screening questions.
Walk-ins may experience longer wait times and availability may vary based on site demand.
The mRNA COVID-19 vaccine series consists of two doses:
• Pfizer-BioNTech (30 µg, 0.3 ml each): 21 days apart
• Moderna (100 mcg, 0.5 mL each) 28 days apart
Persons should not be scheduled to receive the second dose earlier than recommended (i.e., 3 weeks [Pfizer-BioNTech] or 1 month [Moderna]). However, second doses administered within a grace period of 4 days earlier than the recommended date for the second dose are still considered valid. Doses inadvertently administered earlier than the grace period should not be repeated.
The second dose should be administered as close to the recommended interval as possible. However, if it is not feasible to adhere to the recommended interval, the second dose of Pfizer-BioNTech and Moderna COVID-19 vaccines may be scheduled for administration up to 6 weeks (42 days) after the first dose. There are currently limited data on efficacy of mRNA COVID-19 vaccines administered beyond this window. If the second dose is administered beyond these intervals, there is no need to restart the series. Before heading to a vaccination site please remember to eat and take your medications in case there is a wait time.
What should I have available in order to make an appointment?
To make an appointment, you will need either:
- Access to a computer or smart phone to register for an appointment online.
- Access to a telephone to call the COVID-19 hotline, (909) 387-3911. Operators will assist those who cannot make an appointment online. Wait times may vary.
Whether registering online or via phone, please be prepared to provide details such as your address, phone number, age, and general questions regarding your health.
What items should I bring when I come to my appointment?
- A photo ID that includes your date of birth (driver’s license or passport).
Individuals, who do not have proof of residence in San Bernardino County (physical/digital), MUST provide proof of employment in San Bernardino County.
- An employee ID card or badge, OR
- A signed letter from an employer on company letterhead (physical/digital), OR
- A pay stub with your name (physical/digital)
If this is your second dose appointment, please bring:
- Vaccine record card and a photo ID
TRANSPORTATION SERVICES
Omnitrans
Omnitrans is offering two free shuttle services to those with COVID-19 vaccine appointments in Ontario and San Bernardino. One shuttle operates between the Montclair Transit Center and the Ontario Convention Center. Another shuttle operates between the San Bernardino Transit Center and Arroyo Valley High School.

The San Bernardino County Department of Aging and Adult Services (DAAS) has partnered with Omnitrans to offer free transportation to eligible individuals. OmniAccess buses are ADA accessible and accommodate wheelchairs and other mobility devices. Service is available from your home directly to the vaccination site.
Call DAAS at 888-743-1485 to schedule your vaccine appointment and they will reserve your OmniAccess trip at the same time. More information here.
Victor Valley Transit Authority
Victor Valley Transit Authority is providing a free door-to-door shuttle service called the Vaccine Xpress for residents 65 and older with vaccine appointments at Hook Community Center in Victorville or Our Lady of the Desert Catholic Church in the Town of Apple Valley. Rides are by appointment only.
