Vaccine Locations & Registration

Please review each vaccination site listing for appointment eligibility. Appointments are available to San Bernardino County residents and employees in eligible categories. Do not attempt to register if you do not meet the vaccination site qualifications. Appointments are Park and Walk In.


What should I have available in order to make an appointment?

To make an appointment, you will need either:

  • Access to a computer or smart phone to register for an appointment online.
  • Access to a telephone to call the COVID-19 hotline, (909) 387-3911. Operators will assist those who cannot make an appointment online. Wait times may vary.

Whether registering online or via phone, please be prepared to provide details such as your address, phone number, age, and general questions regarding your health.

What items should I bring when I come to my appointment?

  • A photo ID that includes your date of birth (driver’s license or passport).

Individuals being vaccinated MUST provide proof of vaccine eligibility under the current phases and tiers.

  • Proof of residence (physical/digital)
  • An employee ID card or badge, OR
  • A signed letter from an employer on company letterhead (physical/digital), OR
  • A pay stub with your name (physical/digital), OR
  • If eligible due to age, residents can provide their photo ID that includes your date of birth.

If this is your second dose appointment, please bring:

  • Vaccine record card and a photo ID
